HOW TO REPORT A VIOLATION OR LODGE A COMPLAINT

To make a complaint against a liquor or tobacco licensed business, simply call our toll-free hotline, 1-888-838-3956, call the Liquor Enforcement Office nearest you or click here for the online form and give the following information:

  1. Name and address of the business. Example: May Town Tavern, 100 6th Street, Mytown, WA.

  2. Your name, Address and Phone Number. Example: Ms. Amy Smith, 609 6th Street, Mytown, WA. Day Phone: (206) 555-1111 Night Phone: (206) 555-2222.

    You may make an anonymous complaint. However, in order for us to get back to you on the results of the investigation, we need your name and address.

  3. Nature of Complaint: Describe the alleged violation, including the date and time of day. Example: I saw a clerk sell cigarettes and beer to a youth who I know to be underage. This happened at 2:30 PM on Friday, August 14, 1997. I was in the store paying for gas and the clerk didn’t even ask for the youth’s ID.

  4. Description of persons involved. Include their names, if known and a detailed description of the persons. Example: The youth was Jim Brown. I know him from our youth group at church. He is a white male, about age 16. He is about 5’11", 170 lbs. with brown hair and light blue eyes. He brought the beer up to the counter and asked the clerk for a pack of Winstons.

    The clerk had a name badge which said "Keith". Keith was a white male. I would guess he was around forty years old. He was balding with thin blond hair and blue eyes. He was heavy set, about 6' and weighed probably 230 lbs. He got the cigarettes for the youth and did not even ask the youth his age nor did he ask for ID.

Once the complaint is made, it is given to the appropriate Liquor Control Agent, who investigates the complaint within thirty days.
