Out-of-State Wineries/Breweries required forms:

  1. Certificate of Approval (COA)
    1. Master License (available from Department of Licensing).
    2. Non-retail WSLCB Addendum
    3. Monthly reporting forms for Wine/Beer
      1. Wine Monthly Reporting Forms
      2. Beer Monthly Reporting Forms

  2. Current COA holders need to obtain a "COA shipping direct to Washington retailers" endorsement on the "Application for Endorsement" form.

Revised monthly reporting form for Washington wineries and breweries

Retailers required forms:
For Washington retail licensees receiving direct shipments of beer and/or wine from in- or out-of-state U.S. wineries and breweries, the retailer must obtain a “Retailer Receiving Direct Shipments” endorsement using the “Application for Added Endorsement” form.

Note: Retailers must report monthly to WSLCB using form LIQ-871.
