ESB 6537 - Winery to Consumer
Required Forms Page


  1. Wine Shipper Permit forms needed:
    1. Master Application (available from Department of Licensing).
    2. Non-retail WSLCB Addendum
    3. WSLCB Addendum - Beer and Wine Tax Payment Reports.
    4. Copy of Federal Basic permit.
    5. Copy of license from state you are located in.

  2. Endorsement application for COA
    1. Application for Added Endorsement This is a no-fee endorsement. If a winery wants to ship to Washington licensed distributors or retailers, the winery must hold a Certificate of Approval (COA) and must obtain an added endorsement.
    2. WSLCB Addendum - Beer and Wine Tax Payment Reports.
    3. Copy of Federal Basic permit.
    4. Copy of license from state you are located in.

  3. Monthly Reporting forms (to report to WSLCB)
    1. Wine Shippers to Consumers
      Note: This form is only for wine shipper permit holders.
    2. Certificate of Approval (COA) (revised COA form LIQ778, form LIQ778A. Note: If you are a COA you do not need a wine shipper permit.

  4. Required Department of Revenue Forms
    1. Use Tax/Sales Tax Rate Lookup Tool - To determine the correct use tax or sales tax rate to charge a Washington consumer, out-of-state wineries can type in a consumer's address and the appropriate location code and use/sales tax rate will appear.
    2. Beer and Wine Sales to Retailers (Special Notice)
    3. Direct Wine Sales to Consumers (Special Notice)
