The following forms are available for download in a portable document format (PDF). In order to read and print these forms, you must have Adobe Acrobat Reader.

We recommend that you use version 4.0 or 5.0 in order to take full advantage of the features offered on some of the forms. Earlier versions may not successfully work with all forms. A free download is available at:

Downloading Forms to Hard Drive

	Identify the form you wish to download;
	Right click on the form title link;
	Select "Save Target/Link As. . ." when presented with pop-up menu;
	Select the destination directory for the download file; and
	Choose "Save."

Open Form Independent of Internet Browser

Launch the Acrobat Reader application independent of the browser and open the downloaded file.

Note: LCB forms are periodically revised. If you choose to download a form to your hard drive, please be certain to replace every six months by repeating the download process.

Open Form Within Browser Window

Identify the form you wish to open and click on the form title link. Acrobat Reader should immediately launch and the form will be displayed within the browser window. If Adobe Acrobat Reader does not launch and you are prompted with what type of application to open the file with, download a current version of Adobe Acrobat Reader at http://www.adobe.com/products/acrobat/readstep.html.

Completing the Form

	Use either the hand tool or the scroll bar on right side of browser window to move the page around to view all areas.
	Position the hand pointer inside a form field and click.
	Use the I-beam pointer to enter text; or
	Use the arrow pointer to select a check box.
	Press Tab to accept text entered and go to next field.
	Press Shift + Tab to accept text entered and go to previous field.
	Use your mouse to select an area of the form not inside a form field before printing your form.
	Note: Form field contents will not print if the field is active (contains the blinking bar).
